Breathe Easy Program

Our Breathe Easy Program bridges the gap from hospital to home for chronic disease patients. Breathe Easy helps patients minimize hospital readmission, ameliorate quality of life, and reduce exacerbation of chronic respiratory diseases. 

Our Respiratory Therapist works closely with  Pulmonary Doctors and/or Primary Care Physicians to help manage patients’ care, right from their home.

How it works

• A potential patient diagnosed with but not limited to COPD, ALS, Pulmonary Hypertension, Cystic Fibrosis, Pulmonary Fibrosis, Sarcoidosis, Lupus, ARDS, Lung Cancer, Interstitial Lung Disease, Rheumatoid Arthritis, Multiple Sclerosis, Myasthenia Gravis, muscular dystrophy, and heart failure is referred to Carefluent Connect by their doctor or insurance provider.
• The patient is contacted and scheduled for an in- home visit with our respiratory therapist.
• The respiratory therapist delivers and instructs the patient the prescribed equipment (if prescribed), and completes required health assessments. We will also discuss their disease and its process so the patient can better manage their care at home.   
• Follow up patient care is weekly for the first couple of weeks. After care is established and the patient is stable and maintaining good healthy habits, we transition to monthly follow up visits. A health assessment will be performed monthly as part of our care package. We want to make sure we keep our patients healthy and decrease their hospital admission rates.
